A guarantor completes this form to be responsible for the debt of another individual if this individual cannot perform contract obligations. The guarantor has the same responsibilities that the debtor has had and must provide own resources or assets. The guarantor is a co-signer and may be released from the obligations, if the one’s of the debtor are forgiven.

The Guarantor Form is needed, if a debtor cannot personally perform the obligations according to the contract. Remember that the form must be filled out in front of the notary public.
When is the Guarantor Form Due?
The Guarantor Form does not have a deadline. Everything depends on the situation and the agreement the debtor has signed before applying this document.
Is the Guarantor Form Accompanied by Other Documents?
Yes. It may be required to attach a contract or agreement that requires the payment that the debtor cannot afford to make. However, the situations may be different and any other documents may be requested.
What Information do I Include in the Guarantor Form?
The form is only one page. In the form you must indicate the parent or guardian, city and state, student, owner, unit number, full address of the unit, notary public initials and signature. The form includes the information about the guarantor: name, social security number, home telephone, relationship to the applicant, employer and address, position, salary, signature and date.
Where do I Send the Guarantor Form after Completing?
The form must be provided to the creditor or the individual who is responsible for the performance of the payment.
